Location: Various U.S. candidates can be anywhere in the US but maybe asked to do extensive travel or possibly relocate to one of the various bases.

Progressive, employee-friendly OHS is seeking qualified Nurse Case Managers to act as liaisons between patients, their families and health care personnel to ensure efficient and prompt health care service for designated beneficiaries and primary care providers in a clinical setting.

The Nurse Case Manager will provide a full range of professional health nursing principles, practices and procedures in order to analyze the full scope of problems and support functions to ensure effective support of patient care services in clinical settings. The duties performed by the Nurse Case Manager will include, but are not limited to:

Job Duties:
Collect, organize, record, and communicate data relevant to primary health assessments
Assess patients via telephone using established protocols
Establish priorities for patient care, monitor and evaluate progress toward stated goals
Oversee discharge-planning activities
Provide professional assistance to health care finders in order to identify patient’s needs for referrals to appropriate health care providers or facilities
Negotiate, write, finalize, and administer product/service agreements to insure mutual compliance in meeting care goals
Other duties as assigned

Requirements:
Bachelor’s in Nursing
Convey knowledge of Case Management Society of America (CMSA) case management standards and ethics, Utilization Review Accreditation Commission (URAC) case management organizational standards, and Joint Commission for Accreditation of Health Care Organizations (JCAHO) case management standards
Demonstrated skills and knowledge in the following categories:
Choosing, initiating, and modifying selected therapies to optimize patient care
Patient education principles
Management of complex medical, psychological, and financial problems in a clinical setting
Oral and written communication
Team coordination, facilitation, and communication within a group setting
Pre-established utilization review criteria, recognize and report actual or potential quality and risk management issues
VA/DOD Clinical Practice Guidelines
Healthcare financial environment and fiscal dimension of population health
Research tools
Management of information systems and technology
Computers and software applications
Pharmaceuticals, their desired effects, side effects, and complications of their use
Current third party regulations and policies, with special emphasis on TRICARE and Medicare benefits and services

OHS is in an active bidding process for this position and position availability is contingent upon OHS being awarded the contract.
